Septal perforators for an S-GAP flap: Preliminary study. ABSTRACT Perforators of an S-GAP flap have to be dissected through the gluteus maximus muscle. This study evaluate septal perforators running between the gluteus maximus and the gluteus medius muscle. Anatomical study (3 cadavers): in every corps at least 1 septal perforator passing between the gluteus maximus and medius muscle was found (range 0-4). All perforators originated from the anterior branch of the superficial branch of the superior gluteal artery. Echo-doppler study (10 volunteers): at least 1 septal perforator between gluteus maximus and medius muscle was found (range 0-4). Septal perforators could make the S-GAP flap technically easier. A number of septal perforators provides a long pedicle improving the arc of rotation for pedicled flaps or anyway the length of the pedicle for free flaps. To include septal perforators the drawing of the S-GAP flap should be a little higher than conventionally. 48
